Tax Forms

Federal and Wisconsin State Tax Forms have arrived at the Ethel Everhard Memorial Library. Unfortunately, due to a descision made by the IRS Tax Forms Outlet Program (TFOP), the library will not be provided with Federal instruction booklets for the 1040, 1040A, 1040EZ forms.

Message from the IRS TFOP:

"Unfortunately, we are unable to offer Instructions for Forms 1040, 1040A and 1040EZ. Your patrons can obtain copies of Instructions through:

  • IRS.gov/Forms - to view and download
  • IRS.gov/orderforms - to order tax products to be delivered by mail
  • 1-800-829-3676 - to order tax products to be delivered by mail

The decision to reduce the number of tax products available to our TFOP partners was not made lightly.  We realize this decision is not ideal and we understand it may impact you and your customers."

Federal Tax Forms at the library:

1040, 1040A, & 1040EZ

Wisconsin Tax Forms & Instructions at the library:

Form 1, Form 1A, Form WI-Z, Form 1NPR, Schedule WD, & Schedule H & H-EZ (Homestead) with Rent Certificate

Blu-Ray Has Arrived!

In an effort to provide new services and materials to the Westfield community the Westfield Public Library will be adding Blu-Ray movies to its collection. The library will start with a small collection to gauge public interest, but welcomes any Blu-Ray donations as we try to grow our collection.

LEGOs @ the Library

LEGOs @ the Library meets the third Tuesday of each month from 6-7PM. The members use LEGO's to create interesting creations based on a theme that they vote on.  March's theme was "Naruto," a popular graphic novel series!  Creations are now on display at the library. Call (608) 296-2544 for more information.
